Follow-Up Care
Ongoing monitoring, treatment optimization, and a long-term relationship with a care team that knows you.
What It Is
Sleep apnea is a chronic condition, and effective treatment requires ongoing attention — not just a diagnosis and a prescription. Follow-up care at our practice means regular monitoring of your treatment effectiveness, timely adjustments when things change, and continuous access to support and education.
Too many patients are diagnosed, given a CPAP machine, and never heard from again. That approach leads to poor compliance, unresolved problems, and patients who give up on treatment entirely. Dr. Desilva's follow-up care program is designed to prevent that from happening.
How We Support You Long-Term
Our follow-up care program has four core components, each designed to keep your treatment on track.
Compliance Monitoring
We track your CPAP usage data, mask leak rates, and residual apnea events to ensure your treatment is working as it should. Modern CPAP machines transmit data wirelessly, allowing us to monitor your progress between visits and catch issues early.
Treatment Adjustments
Sleep apnea treatment is not set-it-and-forget-it. Weight changes, aging, medications, and other health shifts can affect your needs. Dr. Desilva regularly reviews your data and adjusts pressure settings, mask type, or treatment approach as needed.
Same Care Team, Every Visit
You will not be shuffled between rotating providers who don't know your history. Dr. Desilva and his team build a long-term relationship with every patient, ensuring continuity of care and a deep understanding of your unique situation.
Ongoing Education & Support
Our weekly CPAP classes and Snore No More support groups are available to you throughout your treatment journey — not just when you first start. Questions that come up months or years into therapy are just as important as day-one questions.
What to Expect
01Regular Check-Ins
Scheduled follow-up visits — in-person or via telehealth — to review your compliance data, discuss how you are feeling, and address any concerns. Visit frequency is tailored to your needs and treatment stage.
02Data-Driven Decisions
Every adjustment to your treatment is based on objective data from your CPAP machine, combined with your reported symptoms and quality of life. No guesswork.
03Proactive Outreach
If your compliance data shows a problem — like increasing mask leaks or declining usage — our team reaches out to you before your next scheduled visit. We don't wait for things to get worse.
04Coordinated Care
Dr. Desilva communicates with your primary care physician and other specialists to ensure your sleep treatment is integrated with your overall health management.
Why Follow-Up Care Matters
Research consistently shows that ongoing follow-up care is one of the strongest predictors of long-term treatment success. Here is why:
- Patients who receive regular follow-up care have significantly higher CPAP compliance rates
- Ongoing monitoring catches equipment issues, mask fit problems, and pressure changes early
- Treatment adjustments based on data improve both sleep quality and daytime functioning
- A consistent provider relationship means your doctor understands your complete history
- Continued access to education and support groups reduces frustration and abandonment
A Long-Term Relationship, Not Rotating Providers
One of the most common frustrations patients share with us is being seen by a different provider every time they visit another practice. Each new provider starts from scratch, asks the same questions, and may not know the context of previous decisions.
At our practice, Dr. Desilva and his core team follow you throughout your entire treatment journey. We remember your history, understand your preferences, and build on what we have learned together over time. This continuity is not a luxury — it is a fundamental part of good chronic disease management.
Who Is This For?
Follow-up care is for every patient receiving treatment for sleep apnea or other sleep disorders. Whether you are newly diagnosed and starting CPAP, an established patient needing a treatment adjustment, or someone who has struggled with compliance at another practice, our follow-up care program is built to support you for the long term.
Your Treatment Doesn't End at Diagnosis
Ongoing support and monitoring are the keys to long-term success. Schedule a follow-up appointment with Dr. Desilva's team.
